The application process will include
an
“Application for Admission”, Health Screening form, General
Program Rules, and a financial form. Be sure to read everything
carefully and thoroughly. Please complete and return all forms
except the
Health Screening form. Upon
completion of the application you will need to contact us for an
interview at which time you will be advised whether or not to
begin your medical testing. Please be aware that if your
application is not completely filled out it will be returned to
you which would cause a delay in your being accepted into Teen
Challenge.
No mood altering medication will be allowed for any purpose.
Non-psychiatric drugs prescribed by a doctor for physiological
reasons (such as epilepsy, blood pressure, diabetes, etc.) will
be administered by staff while you are in the program. If you
have a severe case of Hepatitis C requiring you to be on
interferon or a similar drug, we will not be able to accept you
into our program. However, there may be other Teen Challenge
Centers who may be able to accept you.
The following tests are listed on the Health Screening form
and must be completed: S.T.D., Hepatitis A,B, and C Panel,
H.I.V., T.B. Test, Urinalysis, (pregnancy and Pap Smear for
female applicants) and Dental exam. We must have the results
of the above tests and a copy of each lab report, with the
doctor's signature on the back of the Health Screening form. A
dentist's report must also be sent indicating that the dentist
does not foresee any work that will need to be done in the next
year. A T.B. Test is required and must be done no more than
30 days before entering Teen Challenge.
Once we receive the medical results and you are accepted, we
will put your name on our waiting list.
